Predictors of the change in bilirubin levels over twelve weeks of treatment with atazanavir

چکیده
Introduction Atazanavir (ATV) is a protease inhibitor used in the treatment of HIV infection. It is useful in patients on methadone replacement therapy as its once daily dosing facilitates co-administration with methadone and, unlike the non-nucleoside reverse transcriptase inhibitor, efavirenz, it does not accelerate the metabolism of methadone via induction of cytochrome P450 enzymes [1]. ATV is associated with unconjugated hyperbilirubinaemia in 6-40% of patients, overt jaundice in 7-8% and discontinuation in up to 2% [2,3]. The pathophysiology of ATVhyperbilirubinaemia is analogous to Gilbert’s syndrome; ATV competitively inhibits UDP-glucuronyltransferase (UGT) enzymes leading to reduced glucuronidation of bilirubin and increased levels of unconjugated bilirubin [4,5]. Patients with the UGT1A1*28 genotype are particularly, but not exclusively, vulnerable [6,7]. Our study aimed to determine the clinical predictors of ATV-associated hyperbilirubinaemia in our patient population.

Correlations between atazanavir Ctrough and hyperbilirubinemia: a case report
INTRODUCTION Hyperbilirubinemia is a common side effect of the antiretroviral agent atazanavir but is generally reversible upon discontinuation of treatment. We used therapeutic drug monitoring to investigate the occurrence of hyperbilirubinemia in a 49-year-old Hispanic man infected with HIV, following an overdose of ritonavir in ritonavir-boosted atazanavir therapy. CASE PRESENTATION A 49-y...

Background: The impact of boosted protease inhibitor therapy on inflammatory and cardiovascular biomarker levels in treatment-naive HIV-infected patients remains unclear and may differ between agents. Unconjugated bilirubin elevation, which favourably affects vascular biomarkers and cardiovascular disease risk in Gilbert’s syndrome, occurs with atazanavir. Methods: CASTLE was a 96-week study co...

Serum levels of glucose, bilirubin, calcium, phosphorus, protein and albumin were measured in a group of nine healthy men before, and on the first, 10th, 20th and 29th day of Rarnadan, and four weeks after Islamic fasting was terminated. There was a significant reduction in body weight, a significant increase in serum bilirubin and a slight decrease in calcium during Ramadan. Serum glucose decr...
متن کاملConflicting effects of atazanavir therapy on atherosclerotic risk factors in stable HIV patients: A randomized trial of regimen switch to atazanavir
Bilirubin acts as a potent endogenous antioxidant, with higher concentrations associated with lower rates of CVD; the antiretroviral drug atazanavir (ATV) increases bilirubin levels but may also increase von Willebrand factor levels. We tested the hypothesis that increasing endogenous bilirubin using ATV would improve cardiometabolic risk factors and vascular function in older patients with HIV...
